Accurate , up to date , health and social services information intuitively organized, easily accessible and at the fingertips of consumers and providers in Champlain. Topics include: seniors, women's health,mental illness, end of life, children services. |

Our mission is to deliver a seamless experience through the health system for people in our diverse communities, providing equitable access, individualized care coordination and quality health care.
The Community Care Access Centre (CCAC) provides coordination and links to many services that assist people maintain their independence and stay at home as long as possible. Whether you are in the hospital or in your own home the health care professional Case Manager at the CCAC will work with you to determine what services you need, and will then arrange and coordinate them for you. These may include: Nursing,Physiotherapy,Speech and Language Therapy, Homemaking,Equipment,Occupational Therapy,Social Work, Nutrition, Medical and Dressing Supplies, Drug Benefits. These services are funded through the Ministry of Health and Long Term Care. The Case Manager can also connect you with community support services or arrange them on your behalf.
Making a decision to move from your home is not an easy one. It is a time of many questions and emotions and a Case Manager can assist by listening and giving you information you need to make important care decisions. Housing an indoor ice pad, with bleacher seating up to 1000, this facility also houses a lounge and two full kitchens. Both the lounge and the ice surface (summer months) are available for rental for all special occasions. Please contact the municipal office for rental rate and details. Situated on Farmer Road in Combermere, the Community Centre has a fully serviced kitchen bar and stage making it the perfect venue for smaller functions. The centre can also boast the area's only "spring" dance floor! Also available on site is an outdoor ice rink, used in the winter by many families and groups free of charge, and a ballfield. The Combermere Community Centre is home to the CO Blitz, Combermere's annual celebration, held on the Sunday of the August long weekend. Call the municipal office for more information, including rental rates. |

The centre is open every day with the exception of Sundays from 10.00 am. to 4.00 pm. Members are welcome to drop in for a chat, cup of coffee, a game of cards, or to use the library or computer. A volunteer host is on hand while the centre is open.
In addition, there is a full program of activities throughout the week, an active travel program featuring bus trips and our ever popular members’ dinner and entertainment which is also open to guests.
